• Swim Central Water Safety Education Program Registration Form (Click to download)
• Liability Release Form (Click to download)

Step 4: Email the required documents to:

• Government-issued Identification
• Income tax return from the previous year, W-2 or 1099
• Completed CSC Sliding Fee Scale Assessment Form
• Completed Swim Central Water Safety Education Program Registration Form
• Completed Liability Release Form

Step 5: Await program approval from City of Oakland Park.
